Scholarship Award of UCP Chicago Women's
Board
Procedures
In order to allow sufficient time for all eligible students to complete
the application process and for the decision committee to conduct
a thorough and proper review of the applications and choose the
awardees, the deadline for the applications will be May 16, 2007.
The desired time frame of the scholarship award is structured such
that the student can apply the award to the beginning of his/her
2006 school year.
The following sections outline the applicants' eligibility requirements
and application requirements.

| 1. |
Demonstrate proficient use of assistive
technology in a school setting |
| 2. |
Be enrolled in a high school affiliated
with UCP Chicago (through Infinitec Coalitions, etc.) or student
received training/therapy from UCP Chicago/Infinitec. |
| 3. |
Have been accepted to an Illinois college. |
| 4. |
Maintain C+ grade point average. |
| 5. |
Exhibit financial need. |
| 6. |
Submit quarterly reports containing
grades, projects, clubs, etc. |

| The following items must
accompany the completed scholarship application form for consideration: |
| 1. |
Copy of transcripts from high school. |
| 2. |
Copy of the admissions acceptance letter
from the college. |
| 3. |
Three letters of reference (at least
one teacher plus school official, community leader,etc.) |
| 4. |
Copy of 2006-2007 university/college
financial aid offer letter and complete copy of 2006-2007 Student
Aid Report (SAR). If Federal funding not applied for, then a
copy of 2006 Federal Income tax statement. |

Signature card indicating student agrees
to submit future college reports containing grades, projects,
clubs, etc. |

If you do not include your college aid letter
and SAR or tax return, your application will not be considered.
